Standard verification process (NOT high-risk like plumbing/electrical/locksmiths/garage door — so video interviews are not required for the standard process). Required documentation: NPI per provider on profile, state medical license per provider with expiration, business license, BOTH General Liability AND Professional Liability (malpractice) insurance, background checks for owners (Google covers cost via Pinkerton), and a verified Google Business Profile (mandatory for LSA since November 2024). Annual license/insurance renewal required.
